  • Non-clinical performance training services

  • Licensed physical therapy services (where available)

We are committed to protecting your personal information and, where applicable, your protected health information (“PHI”) in accordance with applicable laws, including the Health Insurance Portability and Accountability Act (“HIPAA”).

  • Name, email, phone number

  • Account or inquiry details

  • Communications with us

  • Movement assessments

  • Training data and goals

When collected as part of physical therapy services, this information may constitute PHI.

  • Communicate with you

  • Deliver programs and track progress

  • Schedule and manage appointments

  • Maintain records and comply with legal obligations

We do not sell your personal or health information.

4. HIPAA & Protected Health Information (PHI)

When you engage in licensed physical therapy services, we may collect and maintain PHI.

In those cases:

  • Your PHI is used only for treatment, payment, and healthcare operations

  • We implement safeguards consistent with HIPAA requirements

  • You may receive a separate Notice of Privacy Practices (NPP) if required

Important:
Not all interactions with Applied Physiologics are covered by HIPAA.

  • Website forms, email, and general inquiries are not guaranteed HIPAA-compliant communication channels

  • Non-clinical performance training services are not healthcare services and may not be subject to HIPAA protections.

5. Communication & Platform Use

We may use third-party platforms for:

  • Scheduling

  • Messaging

  • Program delivery

  • Payments

While we select reputable providers, not all platforms are HIPAA-compliant unless explicitly stated.

You acknowledge that:

  • Non-secure communications (e.g., standard email) carry inherent risk

  • You may request more secure communication methods where available

We only share information when necessary:

  • With service providers (e.g., scheduling, hosting, software platforms)

  • To comply with legal obligations

  • To protect rights, safety, or prevent fraud

Where required, we maintain Business Associate Agreements (BAAs) with HIPAA-relevant vendors.

However, no system is completely secure.
